How to Fill Out and Submit Fillable Forms
The way a form opens depends on your device and settings. Usually the form will open directly in your browser. It may instead download and open in a PDF reader if:
- You have installed a PDF reader application on your device (such as Adobe Acrobat)
- You have set that PDFreader as the default application for PDF files\
- Your browser is configured to download PDF files instead of displaying them
Filling Out the Form
The steps are the same regardless of whether the form opens in your browser or in a PDF reader:.
- Type the information into each field.
- Some fields are marked as requered fields, so it is best to complete all the fields.
- Please enter dates in the recommended format: mm/dd/yyyy.
Submitting the Form
If the form opens in your browser:
You may choose either method:

Print and mail the completed form. Mail to: Jessica Hopkins c/o Hennessy Property Management 181 Knight Street Warwick, RI 02886

Save and email the completed form. Email to: jessica@thehennessygrp.com
To save the form as a PDF:

Click the Save icon in your browser’s PDF toolbar, or

Click Print and choose Print to PDF (if supported by your operating system)
If the form opens in a PDF reader application
You can use all the browser options above, plus additional features your PDF reader may provide.
Many PDF readers (including Adobe Acrobat) include a Share button. This option:
- Creates a copy of your completed form
- Lets you choose an email share method
- Opens your email app with either a PDF attachment or a secure link to your completed form
Recipients can open the attachment or click the secure link to view or download your completed form.
